Notification of Closure: Root to Rise Wellness Center
Dear Valued Client & Friend,
It is with a heavy heart that I write to inform you of the impending closure of Root to Rise Wellness Center. After painstaking consideration, we have come to the difficult conclusion that we must cease operations as is and close our location on Market Street effective Wednesday, May 29th, 2024.
This decision was not made lightly, as we understand the impact it may have on you, our dear community. The closure of this location is made necessary after thorough evaluation of various factors, including changes in the healthcare landscape, economic challenges, and the personal circumstances of our team. We want to express our heartfelt gratitude for entrusting us with your healthcare and wellness needs over the past several years. It has been our privilege and honor to serve you and your families, and we are profoundly grateful for your loyalty and support.
We understand that transitioning to a new healthcare provider can be unsettling, and we are committed to making this process as smooth as possible for you. To assist with continuity of care, we will be providing guidance on transferring your medical records to a new healthcare provider. Additionally, if you have any pending appointments or ongoing treatments, we will work with you to ensure proper continuity of care or to assist you in completion or referring to another healthcare provider as necessary. Our staff will be available to answer any questions you may have and to facilitate this transition.
Many of the services we provide such as those by the independent providers Laura Koratsis, Maigan Brewer-Button, Stefanie Holton, and Shannah Warwick will be available through alternate locations and further details can be obtained through them individually. Please be on the lookout for updates to each of their individual practices. Jennifer Share will be offering services through a community health care center and availability can be requested through The Chautauqua Center in Elmira, NY.
Again, we want to express our deepest appreciation for allowing us to be a part of your wellness journey. It has been a privilege to serve you, and we will always cherish the relationships we have built with each and every one of you.
